What to bring to your first office visit:

1. Completed medical questionnaire (PDF) includes:
   
• Registration Form
• Patient Health History / Questionnaire
• Physician Information Sheet (Referring Physician, Primary Physician)
• HIPAA Signature Page
• Medical Records Release Authorization
• Advance Directive for Health Care
• Payment Policy
2. Insurance information
3. Medication List
4. Hospital records if available (ie: medications upon discharge, pathology report, MRI and CT reports)
5. All MRI or CTs images on discs, with reports
6. List of all your physicians
7. A family member or care taker that will be involved in your care
